Frei nach der \u201eSendung mit der Maus\u201c und f\u00fcr alle Nicht-Star-Trek-Fans: \u201e\u201aNuqneH!\u2018 &#8211; das war \u201aGuten Tag\u2018 auf Klingonisch.\u201c<\/p>\n<p>Um der Wahrheit die Ehre zu geben: \u201eNuqneH\u201c ist nur der n\u00e4chstbeste Ausdruck, der auf Klingonisch an \u201eGuten Tag\u201c herankommt und bedeutet in der Sprache der kriegerisch-m\u00fcrrischen Klingonen so etwas wie \u201eWas willst Du?\u201c.<\/p>\n<p>Ja, bei Sprache kommt es schon ein wenig auf die Feinheiten an. Bevor Sie sich fragen, ob Sie sich in der Kolumne geirrt haben und weiterziehen, um die \u201eDigital Health Notizen\u201c zu suchen, lassen Sie mich kurz ausholen: Star Trek hat mich schon in meiner Jugend fasziniert. Unter anderem, weil die Science-Fiction-Serie mindestens genauso gro\u00dfen Wert auf Science, also wissenschaftliche Akkuratesse, wie auf einfallsreiche Fiktion setzte. Und wo die Grenzen der Wissenschaft zum damaligen Zeitpunkt erreicht waren, f\u00fchrte man kurzerhand die L\u00f6sung einfach als gegeben ein. In der Realit\u00e4t steht dem ber\u00fchmten Ort-zu-Ort-Transport (\u201eBeamen\u201c) etwa physikalisch die Heisenbergsche Unsch\u00e4rferelation im Weg &#8211; deshalb gab es in jedem Transporter auch: Einen Heisenberg-Kompensator! Um die Versorgung auf den Raumschiffen sicherzustellen, gab es nicht etwa Frachtr\u00e4ume voller Lebensmittel, sondern \u201eReplikatoren\u201c, die jedes Gericht aus einzelnen Molek\u00fclen zusammensetzen konnten. Und wenn die Crew auf eine fremde Spezies stie\u00df, musste noch nicht einmal untertitelt werden, denn: An jeder Uniform befand sich ein \u201eUniversal Translator\u201c, der simultan in und aus jeder Sprache \u00fcbersetzen und neue Sprachen selbstst\u00e4ndig erkennen und erlernen konnte. Schon in der fr\u00fchen Science Fiction war den Machern bewusst, wie wichtig Sprache f\u00fcr jedes wie auch immer geartete Miteinander ist.<\/p>\n<p>Zumindest hier wurde die Fiction inzwischen von der Science eingeholt: Googles \u201eTranslate\u201c, Apples \u201e\u00dcbersetzer\u201c und eine un\u00fcberschaubare Anzahl von \u00dcbersetzungsdiensten bieten inzwischen so etwas wie einen \u201eKonversations-Modus\u201c an, mit dem man Alltagsgespr\u00e4che halbwegs unfallfrei, wenn auch nicht perfekt, zweisprachig f\u00fchren kann. Doch &#8211; und damit w\u00e4ren wir beim Thema &#8211; nicht immer gen\u00fcgt \u201ehalbwegs unfallfrei\u201c, zum Beispiel bei Arztbesuchen oder in der Pflege. Bei einem Arztbesuch m\u00f6chte das Praxisteam bestm\u00f6glich weiterhelfen k\u00f6nnen, \u00c4rztinnen und \u00c4rzte so gut wie m\u00f6glich Anamnese und Beschwerden verstehen und die Patientinnen und Patienten diese so klar wie m\u00f6glich artikulieren k\u00f6nnen. In zahlreichen F\u00e4llen ist das aber nicht problemlos m\u00f6glich, sei es auf Grund von kognitiven Einschr\u00e4nkungen oder einfach, weil ein:e Patient:in nicht gut genug geschweige denn flie\u00dfend Deutsch spricht. Laut \u201eBusiness Insider\u201c sprechen mehr als 4 Millionen Menschen in Deutschland zu Hause gar kein Deutsch &#8211; die am h\u00e4ufigsten gesprochenen Sprachen sind in diesen F\u00e4llen T\u00fcrkisch, Russisch, Arabisch, Polnisch und Englisch.<\/p>\n<p>W\u00e4hrend man in den meisten Praxen mit Englisch noch einigerma\u00dfen durchkommen sollte, muss man sich in den vielen anderen F\u00e4llen h\u00e4ufig anderweitig behelfen: Kinder oder andere Verwandte mit Deutschkenntnissen, manchmal auch Kolleg:innen, werden dann als \u00dcbersetzer eingespannt. Aus eigenen Gespr\u00e4chen mit z. B. Diabetologen wei\u00df ich: Das klappt dann \u201emal so, mal so\u201c, denn wie gesagt: Es kommt auf die Nuancen an. Wenn es beispielsweise um ein passendes Fr\u00fchst\u00fcck geht, ist der Unterschied zwischen \u201eM\u00fcsli\u201c und \u201eCornflakes\u201c f\u00fcr eine:n Diabetiker:in durchaus von Bedeutung.<\/p>\n<p>Solche Sprachbarrieren sind in der Versorgungsrealit\u00e4t mehr als \u201einteressante Anekdoten\u201c, sie haben reale Auswirkungen: Auf der Seite der Beauftragten der Bundesregierung f\u00fcr Migration, Fl\u00fcchtlinge und Integration etwa wird ausgef\u00fchrt, dass sich Menschen mangels Deutschkenntnissen z. B. eher an Notfallambulanzen wenden, obwohl sie zu einem Haus- oder Facharzt m\u00fcssten. Und dass Sprachbarrieren mithin ernsten Folgen f\u00fcr ihre Gesundheit haben. Eine Studie im kanadischen Ottawa zeigte etwa, dass im Pflegebereich das Risiko f\u00fcr Nebenwirkungen oder Komplikationen bei Patient:innen mit mindestens zwei Vorerkrankungen um 74% geringer war, wenn sowohl die Patient:innen als auch die Pflegekr\u00e4fte und \u00c4rzt:innen dieselbe Sprache sprachen.<br \/>\nUnd jetzt? Zun\u00e4chst einmal: Das Problem ist bekannt und wird auch politisch unter dem Begriff der \u201eSprachmittlung in der Gesundheitsversorgung\u201c diskutiert. Die Herausforderung: Bei gesch\u00e4tzten 800.000 bis 1.000.000 n\u00f6tigen Eins\u00e4tzen pro Jahr f\u00fcr eine solche \u201eSprachmittlung\u201c muss auch jemand die Kosten \u00fcbernehmen &#8211; und zumindest die GKV schlie\u00dft jetzt und f\u00fcr die Zukunft die \u00dcbernahme solcher Kosten aus. Auf der anderen Seite besteht beispielsweise der Bundesverband der Dolmetscher und \u00dcbersetzer (BD\u00dc) darauf, dass solche Leistungen nur von offiziell zugelassenen Sprachmittlern erbracht werden k\u00f6nnen, am besten vor Ort. Das klingt &#8211; ganz offen gestanden \u2013 unbezahlbar, und vor allem noch viel weniger organisierbar. Wenn der BD\u00dc beispielsweise auf die Bedeutung von Gef\u00fchlen und kulturellen Besonderheiten hinweist, verstehe ich das. Wenn Video-Telefonie dann aber nur als Notl\u00f6sung, die einen Vor-Ort-Einsatz nie ganz ersetzen kann, bezeichnet wird, ist mein Verst\u00e4ndnis nicht mehr so wirklich vorhanden. Und wenn der BD\u00dc (zumindest noch im Mai 2023) kategorisch ausschlie\u00dft, dass f\u00fcr die Sprachmittlung auf maschinelle \u00dcbersetzung zur\u00fcckgegriffen werden k\u00f6nnte, wittere ich &#8211; verst\u00e4ndlicherweise &#8211; nur noch Lobbygeruch. Denn dass Medizin-spezifische Sprachmodelle (LLMs, Large Language Models) f\u00fcr dieses Problem eine der haupts\u00e4chlichen und tragenden L\u00f6sungen sein werden, d\u00fcrfte inzwischen wirklich au\u00dfer Frage stehen &#8211; selbst wenn man Erschwernisse wie kulturelle Besonderheiten mit einbezieht.<\/p>\n<p>\u00dcbrigens: Bei der schieren Menge solcher notwendiger \u201eSprachmittlungen\u201c d\u00fcrfte sich auch kein einziger professioneller Sprachmittler Sorgen um die eigene berufliche Zukunft machen d\u00fcrfen. Und bis es soweit ist? So lange gibt es nat\u00fcrlich bereits andere, ebenfalls funktionierende, aber eben oft analoge Hilfsmittel. Die Stadt M\u00fcnchen beispielsweise setzt zur \u00dcberwindung von Sprachbarrieren auf: Ausmalkarten. Not macht eben erfinderisch.<\/p>\n<p>Bei uns gibt es jetzt Abendbrot &#8211; bei der aktuellen Jugendsprache durchaus auch ein Fall f\u00fcr einen Sprachmittler.
