Nat\u00fcrlich h\u00e4nge auch ich ab und an meinen Gedanken nach, aber doch nicht w\u00e4hrend ich\u00a0warte. Warten ist bei mir etwas sehr Aktives: Ich habe ganz aktiv Stress vor schwierigen oder potenziell negativen Terminen. Ich besch\u00e4ftige mich ganz aktiv auch noch Tage nach jedem Online-Kauf mit dem erstandenen Objekt der Begierde, so lange, bis es mir von einem Paketboten, den ich zuvor auf einer Stra\u00dfenkarte Meter f\u00fcr Meter verfolgt habe, in die ungeduldig zitternden Arme gelegt wird. Ich gr\u00fcble und recherchiere ganz aktiv zu jedem Wehwehchen, von der ersten Google-Suche bis zum Arzttermin. Die Zeit zwischen dem angespannten \u201eJetzt\u201c und diesem &#8211; positiven oder negativen &#8211; \u201eZiel in der Zukunft\u201e, die will prall gef\u00fcllt sein. Sei das Dr. Google, der mir schon vor meinem Routine-Check-Up mein k\u00f6rperliches Alter verr\u00e4t. Seien das Amazon-Rezensionen, die mich noch vor Eintreffen meines neuen Smartphones wieder und wieder best\u00e4rken, die richtige Kaufentscheidung getroffen zu haben. Sei es ChatGPT, das mir vor einem ernsten Gespr\u00e4ch noch ein paar weitere Argumente mit an die Hand gibt. \u201eVorfreude ist die sch\u00f6nste Freude\u201c? Ein f\u00fcr mich unfassbarer Unsinn.<\/p>\n<p>Und jetzt stellen Sie sich vor, wie unertr\u00e4glich das Warten ist, wenn es nicht um solche Banalit\u00e4ten\u00a0geht. Wenn Sie nicht auf das Eintreffen eines Impulskaufes warten. Wenn es nicht um einen Routine-Check-Up geht. Stellen Sie sich vor, Sie f\u00fchlen sich leer, hoffnungs- und antriebslos, suchen jeden Tag nach einem Grund aufzustehen, finden aber keinen. Sagen wir also: Sie k\u00e4mpfen mit einem echten Problem geistiger Gesundheit, oder neudeutsch: Mental Health.\u00a0Sicher: Anlaufstellen f\u00fcr den Notfall gibt es &#8211; von telefonischen und Online-Angeboten bis zu Notfallambulanzen. Doch danach beginnt &#8211; nicht nur hierzulande &#8211; in der Regel ein langes Warten.<\/p>\n<p>Bundesweit warten Patient:innen auf einen Psychotherapie-Platz im Durchschnitt etwa 140 Tage. In besonders gut versorgten Ballungszentren wie M\u00fcnchen auch mal k\u00fcrzer, um die 80 Tage. Das klingt auch ohne gro\u00dfes Hin- und Her-Rechnen erst einmal \u201elang\u201c &#8211; doch machen wir uns bewusst: 140 Tage, bzw. 20 Wochen, bzw. 5 Monate warten Patient:innen im Schnitt auf einen Therapieplatz. Und: Mental Health Erkrankungen sind kein Randph\u00e4nomen &#8211; weder in Deutschland, noch in der EU, noch weltweit. In Deutschland waren laut OECD bereits 2019 rund 18% der Deutschen von psychischen Erkrankungen betroffen. Wohlgemerkt: Vor Corona und der damit einhergehenden weiteren Verschlechterung in diesem Bereich.<\/p>\n<p>\u201eEin wichtiges Thema\u201c, m\u00f6gen Sie vielleicht sagen, \u201eaber wieso ist das relevant f\u00fcr die \u201aDigital Health Notizen\u2018?\u201c. Mir liegt dieses Thema aus drei Gr\u00fcnden am Herzen. Zum Einen: Schlichtweg, weil es ein wichtiges und relevantes Thema ist, \u00fcber das man sprechen sollte und muss &#8211; unabh\u00e4ngig ob \u201eDigital\u201c Health oder nicht. Zum Zweiten, weil sich zeigt, dass digitale Angebote im Bereich der Mental Health offenbar von ganz besonderem Nutzen sein k\u00f6nnen. Und zum Dritten, weil ich &#8211; wie so oft an dieser Stelle &#8211; einen Appell zu mehr Pragmatismus mitgebracht habe.<\/p>\n<p>Betrachten wir beispielsweise das DiGA-Verzeichnis des BfArM, also das Verzeichnis der \u201eDigitalen Gesundheitsanwendungen\u201c, die gesetzlich versicherte Patient:innen bei einer entsprechenden Diagnose auf Rezept verordnet und erstattet werden k\u00f6nnen. Das DiGA-Verzeichnis umfasst derzeit 57 solche Anwendungen,\u00a0\u00a022 davon sind \u201evorl\u00e4ufig\u201c gelistet, 35 \u201edauerhaft\u201c &#8211; soll hei\u00dfen: In einer klinischen Studie wurde ein positiver medizinischer Versorgungseffekt nachgewiesen. Diese 57 Anwendungen gliedern sich in 12 Kategorien &#8211; von A wie Atemwege bis zu V wie Verdauung. In die Kategorie \u201ePsyche\u201c fallen von 57 DiGAs: 27, also 47% oder eben Knapp die H\u00e4lfte. Und 18 davon, also genau zwei Drittel, sind bereits dauerhaft gelistet. Was sagen uns diese Zahlenspiele also? Zum einen ganz profan: Psychische Erkrankungen haben offenbar genug Relevanz, dass sich eine Vielzahl von DiGA-Herstellern damit befasst und darin eine Gelegenheit sieht, Geld zu verdienen. Zum anderen aber auch: Es ist f\u00fcr zwei Drittel dieser Anwendungen bereits klinisch erwiesen, dass Sie auch tats\u00e4chlich bei der Behandlung psychischer Probleme unterst\u00fctzen k\u00f6nnen, von der Hilfe bei der Rauchentw\u00f6hnung, der Behandlung von Panik-Attacken und Depressionen, bis zu Ess-, Angst-, und Schlafst\u00f6rungen.<\/p>\n<p>Wenn man sich den aktuellen DiGA-Report zu Gem\u00fcte f\u00fchrt, stellt man erfreut fest: Insbesondere Apps aus der Kategorie \u201ePsyche\u201c werden bereits h\u00e4ufig verschrieben: Jedes dritte eingel\u00f6ste DiGA-Rezept fiel bisher in diese Kategorie. Aber: In absoluten Zahlen waren das von September 2020 bis September 2023 gerade einmal 121.000 eingel\u00f6ste Rezepte f\u00fcr eine DiGA aus dem Bereich \u201ePsyche\u201c. Zur Erinnerung: Fast jede:r f\u00fcnfte Deutsche hat laut OECD eine psychische Erkrankung. Und &#8211; wie auch in vielen anderen L\u00e4ndern &#8211; der Zugang zu Hilfe ist zwar grunds\u00e4tzlich vorhanden, Patient:innen m\u00fcssen aber oft lange Wartezeiten \u00fcberbr\u00fccken, in denen die ein oder andere DiGA vielleicht schon helfen k\u00f6nnte. Daher meine Bitte: Denken wir auch hier pragmatisch &#8211; nat\u00fcrlich ersetzt eine App keinen pers\u00f6nlichen Therapeuten. Doch f\u00fcr die \u00dcberbr\u00fcckung dieser endlosen Wartezeiten haben wir mit DiGAs in vielen F\u00e4llen zumindest oft schon ein &#8211; klinisch gepr\u00fcftes &#8211; n\u00e4chstbestes\u00a0Mittel der Wahl.
