Naja, es ginge vielleicht auch eine Nummer kleiner, aber Sie werden verstehen, was ich meine: Welches Kind hat nicht mit den Augen gerollt, eine Schnute gezogen, gen\u00f6lt oder gar geweint, wenn es hie\u00df: \u201eAus Prinzip.\u201c \u201eWarum nur eine Kugel Eis?\u201c \u201eWarum schon jetzt ins Bett?\u201c \u201eWarum muss ich das von meinem Taschengeld bezahlen?\u201c \u201eWeil jeder nur eine Kugel bekommt.\u201c \u201eWeil wir das so vereinbart haben.\u201c \u201eWeil Du genau daf\u00fcr Taschengeld bekommst.\u201c So viele unbefriedigende Wege, um mir im Grunde nichts anderes zu sagen: \u201eAus Prinzip.\u201c Als Steppke war das eine Nicht-Begr\u00fcndung, ein billiges Abb\u00fcgeln all der Argumente, die f\u00fcr mein Ersuchen gesprochen h\u00e4tten. Ungerecht.<\/p>\n<p>Heute, als langj\u00e4hriger Angestellter dreier S\u00f6hne, ebenso wie als Unternehmer, habe ich verstanden, dass es ziemlich genau umgekehrt ist, dass Prinzipien wertvolle Leitplanken und Grenzen f\u00fcr einen selbst und die Menschen um einen herum sein k\u00f6nnen \u2013 unabdingbar f\u00fcr das gemeinsame Miteinander. Ich wei\u00df z. B. recht genau, mit wem ich <em>nicht<\/em> arbeiten m\u00f6chte. Und unsere Kinder wissen recht genau, was sie von ihren Eltern erwarten k\u00f6nnen. Und ich wei\u00df im Zweifel \u2013 zumindest in den charakterstarken Momenten: \u201eDa musst Du jetzt durch: <em>Hilft ja nichts<\/em>.\u201c<\/p>\n<p>Womit wir bei unserem eigentlichen Thema w\u00e4ren: Hom\u00f6opathie. Keine Sorge, bis auf diesen kleinen Seitenhieb wird das hier kein weiteres \u201eBashing\u201c, davon gibt es wahrlich schon genug und sie f\u00fchren letztlich ja auch nirgendwo hin. Ich m\u00f6chte nicht \u00fcber Wirksamkeit und Unwirksamkeit von Hom\u00f6opathika, \u00fcber Glauben und Nicht-Glauben, Placebo- und dar\u00fcber hinaus gehende Effekte sprechen. Es geht wie gesagt: Ums Prinzip.<\/p>\n<p>Die Ank\u00fcndigung von Gesundheitsminister Karl Lauterbach, Hom\u00f6opathie zuk\u00fcnftig nicht mehr durch gesetzliche Krankenkassen erstatten lassen zu wollen, stie\u00df auf gemischte Reaktionen. Zum einen sind da die altbekannten \u201eHom\u00f6opathie wirkt nicht \u2013 Hom\u00f6opathie wirkt doch\u201c-Lager, die Klassiker. Aber auch laute \u201eHaben wir keine gr\u00f6\u00dferen Probleme im Gesundheitssystem\u201c-Stimmen im Dreiklang mit \u201eDie Einsparungen w\u00e4ren \u2013 wenn \u00fcberhaupt \u2013 marginal.\u201c und \u201eDer Minister mischt sich hier in die Belange der Selbstverwaltung ein.\u201c<\/p>\n<p>Werden wir also <em>grunds\u00e4tzlich: <\/em>Der Auftrag der Gesetzlichen Krankenkassen ist es, \u201edie Gesundheit der Versicherten zu erhalten, wiederherzustellen oder ihren Gesundheitszustand zu verbessern sowie die Versicherten aufzukl\u00e4ren, zu beraten und auf eine gesunde Lebensf\u00fchrung hinzuwirken.\u201c \u2013 finanziert wird das Ganze \u00fcberwiegend aus den Beitr\u00e4gen der Versicherten. Was die Kassen regul\u00e4r erstatten, die <em>Regelleistungen<\/em>, ist klar <em>geregelt<\/em> und unterliegt einer aufw\u00e4ndigen Kosten-Nutzen-Bewertung. Alles dar\u00fcber hinaus \u2013 unter anderem eben auch Hom\u00f6opathie \u2013 f\u00e4llt unter die Kassen-spezifischen <em>Satzungsleistungen<\/em>. Die M\u00f6glichkeit zur Erstattung von Satzungsleistungen gibt es im Sozialgesetzbuch unter anderem, um Wettbewerb unter den Krankenkassen zu erm\u00f6glichen.<\/p>\n<p>Jede Diskussion \u2013 um freiwillige Vorsorge-Leistungen, zus\u00e4tzliche Heilmittel-Erstattungen oder eben auch -Hom\u00f6opathika, sollte sich dementsprechend auch prim\u00e4r mit zwei Fragen besch\u00e4ftigen: Tr\u00e4gt eine Leistung zur besseren Versorgung der Versicherten bei und geht eine Krankenkasse dabei mit ihren finanziellen Mitteln sinnvoll um. Ob eine Einsparung \u201emarginal\u201c w\u00e4re, tut dann nichts zur Sache. Und ob es \u201eBesseres zu tun gibt\u201c, ebenso wenig. Nun bin ich selbst kein Fan von Hom\u00f6opathie, aber ich respektiere selbstredend, wenn sie anderen Menschen hilft. Ich schw\u00f6re bei einer Erk\u00e4ltung oder bei Stress zum Beispiel auf ein sch\u00f6nes Schaumbad mit \u00e4therischen \u00d6len. Beides ist in Ordnung (\u201eWenn\u2019s hilft.\u201c) und <em>keins von beiden sollte von einer Krankenkasse erstattet<\/em> werden.<\/p>\n<p>Nicht sehr digital bisher, diese <em>Digital Health<\/em> Notizen, werden Sie vielleicht denken. Kommt jetzt: Erst in der letzten Ausgabe unserer kleinen Kolumne hatten wir uns mit DiGAs und dem Bericht bzw. der Einordnung des GKV-Spitzenverbandes besch\u00e4ftigt. Der Tenor: Hohe Kosten \u2013 gar ein Finanzierungsrisiko f\u00fcr die GKV \u2013 und angeblich kein wirklich nachgewiesener Nutzen. Da muss was getan werden, damit dieses Konzept in Zukunft \u00fcberhaupt fliegen k\u00f6nnte. Wer gleichzeitig aber Hom\u00f6opathika mit Begr\u00fcndungen wie \u201ewegen hoher Nachfrage der Versicherten\u201c erstattet, dem will ich es nicht so recht abnehmen, dass es bei der Opposition gegen DiGAs dann auf einmal <em>ums Prinzip<\/em>, um \u00a0Kosten- und Nutzenbewertungen geht.<\/p>\n<p>Vergleichen wir kurz: Dr. Natalie Grams, selbst fr\u00fcher Hom\u00f6opathin und heute eines der Aush\u00e4ngeschilder der Hom\u00f6opathie-Gegner, ordnet die Anwender von Hom\u00f6opathika ein als \u201ejung, gesund, naturbezogen und eher gesundheitsbewusst\u201c. 2021 lagen die Ausgaben der GKV f\u00fcr Hom\u00f6opathika bei etwa 22 Millionen Euro, einen klinischen Nachweis der Wirksamkeit gibt es seit 1796 allerdings noch nicht. Und bei DiGAs? 67,5 Millionen Euro hat die GKV 2023 daf\u00fcr ausgegeben \u2013 wobei mehr als die H\u00e4lfte der gelisteten DiGAs inzwischen den klinischen Nachweis eines positiven Versorgungseffektes erbracht haben. Der Rest ist (f\u00fcr ein Jahr) vorl\u00e4ufig gelistet. Das mittlere Alter der DiGA-Nutzer liegt bei 45 Jahren, mit der \u201eh\u00f6chsten Inanspruchnahme\u201c in den \u201eAltersgruppen von 50 bis 60\u201c. Ein Schelm, wer bei diesen Zahlen B\u00f6ses denkt.<\/p>\n<p>Um welches Prinzip geht es also bei all der Kritik an DiGAs? Wenn es \u2013 wie bei Hom\u00f6opathie \u2013 um die Nachfrage der Versicherten geht: Haken dran, die steigt seit Einf\u00fchrung der DiGAs kontinuierlich und Krankenkassen sollten sich gl\u00fccklich sch\u00e4tzen, diese Nachfrage bedienen zu k\u00f6nnen. Geht es darum, f\u00fcr die <em>richtigen <\/em>Versicherten \u2013 mit hohen Beitr\u00e4gen und geringen Kosten \u2013 attraktiv zu sein? Dann w\u00fcrde ich nichts \u00e4ndern. Oder geht es um eine Kosten-Nutzenbewertung? Dann m\u00fcssten die Krankenkassen entweder aufh\u00f6ren, Hom\u00f6opathika zu erstatten \u2013 oder DiGAs einfach ohne Murren erstatten und ihnen statt 12 bis 24 Monate noch rund 200 Jahre Zeit f\u00fcr den Nachweis eines positiven Versorgungseffektes geben.<\/p>\n<p>Ich mache mich jetzt auf den Weg zur Trabrennbahn \u2013 ein bisschen Prinzipien-Reiten \u2013 und danach gibt es \u00a0ein sch\u00f6nes Schaumbad mit \u00e4therischen \u00d6len.
