Eigentlich genau wie vorher in meinen &#8222;Digital Health Notizen&#8220;, nur unter neuem Namen und dadurch eben: Besser.<\/p>\n<p>Bevor sich bei den Stammleser:innen dieser Kolumne gleich tiefe und dauerhafte Verwerfungen auf der Stirn bilden: Nein, keine Angst, die &#8222;Digital Health Notizen&#8220; bleiben nat\u00fcrlich die &#8222;Digital Health Notizen&#8220;. Mal ehrlich: Es w\u00e4re ja weder sinnvoll noch irgendwie glaubw\u00fcrdig, dieselben Inhalte aus einer reinen PR-Motivation heraus einfach unter einem neuen Namen zu publizieren. Wobei ich es mir hier auch nicht zu einfach machen m\u00f6chte: Grunds\u00e4tzlich sind &#8222;Re-brands&#8220; ja nicht pauschal nur nach Aufmerksamkeit heischende &#8222;Mogelpackungen&#8220;. Erst einmal ist ein neuer Name ganz einfach eine Ver\u00e4nderung \u2013 mit vielen m\u00f6glichen Hintergr\u00fcnden und Motivationen. Aus dem Schokoriegel &#8222;Raider&#8220; wurde hierzulande etwa &#8222;Twix&#8220;, um die Marke sukzessive international zu vereinheitlichen. Zumindest ein bisschen um&#8217;s gebeutelte Image d\u00fcrfte es derweil bei der Umfirmierung der &#8222;Hamburg-Mannheimer&#8220; zur &#8222;Ergo&#8220; gegangen sein. &#8222;Super RTL&#8220; hei\u00dft seit August diesen Jahres &#8222;RTL Super&#8220;, um Konsistenz mit anderen &#8222;RTL&#8220;-Pr\u00e4fix-Sendern zu gew\u00e4hrleisten \u2013 und warum das &#8222;D\u00e4nische Bettenlager&#8220; seit einiger Zeit &#8222;JYSK&#8220; hei\u00dft, wird zum einen f\u00fcr alle Zeiten ein ungel\u00f6stes R\u00e4tsel bleiben, ist zwar k\u00fcrzer, aber auch nicht wirklich einpr\u00e4gsamer. Nike hie\u00df einst Blue Ribbon, Google wurde zu Alphabet, der Facebook-Konzern wurde vor noch nicht allzu langer Zeit zu Meta, die Agentur f\u00fcr Arbeit war einst Das Arbeitsamt, und so weiter. Ebenso endlos wie die Reihe der Beispiele ist auch die Motivation: Vom Streben nach frischer Aufmerksamkeit, dem Ausdruck einer neuen inhaltlichen Ausrichtung, \u00fcber die Kommunikation gemeinsamer Werte nach einer Fusion, bis zur Manifestation eines neuen Images.<\/p>\n<p>Was also bedeutet es, wenn um 2025 herum \u2013 wie es in der Pressekommunikation des Bundesgesundheitsministeriums hei\u00dft &#8211; die BZGA, die Bundeszentrale f\u00fcr gesundheitliche Aufkl\u00e4rung, in einer neuen Beh\u00f6rde aufgehen wird: dem &#8222;BIPAM&#8220;, dem &#8222;Bundesinstitut f\u00fcr Pr\u00e4vention und Aufkl\u00e4rung in der Medizin&#8220;? Liest man die Pressetexte, geht es hier tats\u00e4chlich um mehr als den sch\u00f6nen Schein, um echtes &#8222;Nachholen&#8220;. Deutschland gebe &#8222;mehr f\u00fcr Gesundheit aus, als jedes andere EU Land&#8220;, sei aber &#8222;bei der Lebenserwartung trotzdem nur Durchschnitt&#8220;. Die unmissverst\u00e4ndliche Botschaft: &#8222;Bei der Verhinderung von Krankheiten (&#8230;) haben wir Nachholbedarf.&#8220; Der &#8222;Branding&#8220;-Schwenk der BZGA geht also von &#8222;Gesundheitlicher Aufkl\u00e4rung&#8220; &#8211; also einem kommunikativen Fokus \u2013 mit dem BIPAM hin zum tats\u00e4chlichen Ergebnis, zur &#8222;Pr\u00e4vention&#8220;. So kritisch ich mit der Gesundheitspolitik oft ins Gericht zu gehen wei\u00df: Das klingt erstmal gut. Ein Institut mit dem Fokus auf Pr\u00e4vention der gr\u00f6\u00dften Krankheitsgruppen, unter anderem also Krebs-, Demenz- und Herzkreislauf-Erkrankungen. Dass es ganz allgemein erstrebenswerter ist, von keiner dieser Erkrankungen betroffen zu werden, d\u00fcrfte erst einmal unstrittig sein. Auch volkswirtschaftlich ist da wenig Raum zur Diskussion: Allein f\u00fcr 2021 taxiert die BAUA \u2013 die Bundesagentur f\u00fcr Arbeitsschutz und Arbeitsmedizin \u2013 den Ausfall an Bruttowertsch\u00f6pfung durch Krankheitstage auf ca. 153 Mrd. Euro \u2013 etwas weniger als 5% der gesamten deutschen Bruttowertsch\u00f6pfung 2021.<\/p>\n<p>153 Mrd. \u20ac sind ein beachtlicher Schaden \u2013 und das ist nur der Wertsch\u00f6pfungsausfall. Das BMG selbst gibt an, dass in Deutschland gut 5.000 \u20ac pro Person pro Jahr im Gesundheitssystem ausgegeben werden. Machen wir an dieser Stelle kurz Pause: Schnappen Sie sich Zettel und Stift, und sch\u00e4tzen Sie, wieviel die gesetzlichen Krankenversicherungen 2021 pro Person f\u00fcr Pr\u00e4vention ausgegeben haben.<\/p>\n<p>Mal sehen, wie nah Sie dran sind: Laut GKV-Spitzenverband waren es insgesamt 537 Milliar- oh: Millionen Euro. Also 7,34 \u20ac pro versicherter Person. Sp\u00e4testens beim Blick auf solche Zahlen d\u00fcrfte klar sein, dass es mit einem &#8222;BIPAM&#8220; allein wohl nicht getan sein wird \u2013 das w\u00e4re tats\u00e4chlich leider alter Wein in neuen Schl\u00e4uchen. Wenn das lange vernachl\u00e4ssigte Thema Pr\u00e4vention nun wirklich ernsthaft angegangen werden soll, wenn wir wegen des Demographischen Wandels l\u00e4nger arbeiten sollen, bzw. wegen eines permanenten Fachkr\u00e4ftemangels auch m\u00fcssen, dann braucht es nicht nur eine ambitionierte neue Beh\u00f6rde \u2013 dann m\u00fcssen unter anderem auch die Krankenversicherungen fundamental umdenken. Ist es wirklich damit getan, einfach festzustellen, dass man zwar mehr pro versicherter Person f\u00fcr Pr\u00e4vention ausgibt, aber die Inanspruchnahme individueller Pr\u00e4ventionsangebote 2021 im Vergleich zum Vorjahr gesunken ist? Dass Krankenkassen im tiefsten Inneren noch nicht wirklich voll auf\u2019s Pr\u00e4ventionsgas dr\u00fccken, mag historisch bedingt sein: Tats\u00e4chlich haben klassische Pr\u00e4ventionsprogramme oft nicht den besten Ruf \u2013 und die meisten gesetzlichen Krankenkassen ein pr\u00e4ventiv (pardon!) kritisches Verh\u00e4ltnis dazu. Doch allein die Verf\u00fcgbarkeit und vollst\u00e4ndig andere Natur nun verf\u00fcgbarer digitaler Helfer \u2013 etwa in den Bereichen Mental Health oder Fitness \u2013 sollte (schon seit langem) Grund genug sein, diese Vorbehalte auf den Pr\u00fcfstand zu stellen: Digitale Angebote sind so nah an den Patient:innen, und so individuell, wie es klassische Pr\u00e4vention \u2013 vom \u201eRezeptbuch\u201c bis zu \u201eBewegungsempfehlungen\u201c \u2013 schon von Natur aus nie werden sein k\u00f6nnen. Und f\u00fcr die Erhebung der n\u00f6tigen Evidenz zu solchen digitalen Pr\u00e4ventiv-Angeboten (sprich: \u201eWirkt Pr\u00e4vention?\u201c) g\u00e4be es zumindest schon einmal ein \u00fcberaus erfolgreiches Vorbild: Die DiGAs. Auch wenn der Gedanke bei den Krankenkassen nicht zur Herz-Kreislauf-Stabilisierung beitragen wird: Wie w\u00e4re es mit einer Erweiterung des DiGA-Konzeptes auf \u2013 bisher explizit ausgenommene \u2013 digitale Pr\u00e4ventions-Anwendungen: DiPrAs.<\/p>\n<p>Als Branchen-Interessierte kann ich Sie, liebe Leser:innen und Lesser des PM-Reports, nat\u00fcrlich auch nicht einfach so \u201evom Haken lassen\u201c: Pr\u00e4vention ist n\u00e4mlich eine ebenso gro\u00dfe Chance \u2013 und eigentlich auch ein wenig gesamtgesellschaftliche Verpflichtung \u2013 f\u00fcr Pharmaunternehmen. Ja, ich wei\u00df: Dass sich eine Branche, die prim\u00e4r zur Therapie von Erkrankungen beitr\u00e4gt und damit dann eben auch Geld verdient, \u00fcberm\u00e4\u00dfig in der Pr\u00e4vention engagieren soll, ist erstmal \u2013 sagen wir \u201ekonterintuitiv\u201c. Aber: Jetzt, da das Thema offenbar Aufmerksamkeit (und damit wahrscheinlich und hoffentlich eben auch wirtschaftliche Mittel) erh\u00e4lt, wird sich in jedem Fall jemand darum k\u00fcmmern \u2013 warum also nicht auch die Pharmabranche?<\/p>\n<p>Bevor bei mir jetzt allzu gro\u00dfe Ersch\u00f6pfung einsetzt, gehe ich schon einmal \u2013 pr\u00e4ventiv \u2013 in den gem\u00fctlichen Teil meines Abends \u00fcber.
